  1. The transition probabilities between states are shown between 0 and 1, and some probabilities are time dependent (not always fixed). The reference case was a 30-year-old patient and reflected the median patient age in our cohort study (33.0 years old; unpublished data). A 40-year time horizon was selected for modelling. The model cycle time was 1 year, since the monitoring and evaluation of the programme including patients’ treatment outcomes and retentions were conducted annually by the government
